Student Dental Insurance
The student dental insurance plan is administered by United Concordia.
Continuing students must enroll from July 1 to June 30. New students have the option of enrolling during in-person orientation activities.
Coverage will be effective August 1 for students matriculating during the summer or October 1 if you matriculate during 1st term. Students cannot terminate their dental coverage once enrolled.
Deadlines for New Student Enrollment
- Summer deadline: July 7th (with coverage effective date of 08/01/2013)
- Fall deadline: September 8th (with coverage effective date of 10/01/2013)

- Co-payment for each service stated in brochure—no questions on price
- All class I services have $0 co-pay
- $5 co-pay for office visit
- All class II and III have higher co-pays
- No claims—pay at visit
- No deductible or maximum

Plan 2: Access Plan
Access Plan Summary
- No primary dentist—larger network (view list under Concordia Access on United Concordia website)
- All class I services are covered 100% up to $500 maximum annually
- All class II and III are discounted 20–30% off usual cost varying by dentist
- No brochure with pricing—you must contact individual dentist for their rates
- No claims—pay at visit
- $25 annual deductible

Vision Discount Program
Please note that participants in the United Concordia dental insurance plan have access to the Davis Vision Discount Program. This is NOT vision insurance coverage. Davis provides dental plan participants with a low-cost, traditional vision discount program that provides significant discounts on eye exams, lenses, frames and additional eyewear options.
