Diploma Order Form

It is never too soon to begin planning for the next graduation. The data that you supply will be used for your diploma order, the University-wide Commencement program, and the School's Convocation program.

In February, you will be asked to confirm the data supplied on this form. When you receive the first mailing in January, you can edit any special characters or scientific notations.

• Please make sure that between now and graduation, you keep your address current in our system.  Student Data Change Forms are available in the Student Affairs Office, Suite E1002, or by clicking here.
• Only expected May 2007 graduates should complete this form.  Any forms submitted by non-expected graduates will be discarded.
