If you are currently enrolled in DHMO or Access Dental Insurance Plan and would like to continue your enrollment in the dental plan for Academic Year 2013-14, please take a moment to read this very important information. All students currently enrolled in the Dental Insurance Plan will have their plan automatically terminated effective July 1, 2013.Students interested in enrolling in United Concordia dental plan for AY 2013-14 MUST complete the appropriate enrollment form located on this website. Please fax (410) 614-8633, email ekoimur@jhsph.edu or deliver (Suite W1101) your completed form by June 15, 2013. You may obtain more information regarding each plan by visiting our site. If you are graduating or will no longer be a registered student at the Bloomberg School of Public Health you are not eligible for dental coverage. The plans are effective July 1, 2013 through June 30, 2014. You must enroll for the entire year, terminations are not allowed once enrolled. Premiums will be posted to your July 2013 statement and will be due July 20, 2013. Please find the rates listed below: New students have the option of enrolling during in-person registration. Continuing student deadline - June 14, 2013 with coverage effective July 1, 2013. New student deadline - July 12, 2013 with coverage effective August 1, 2013.
