Improving Health Care And Palliative Care For Advanced And Serious Illness

A review of interventions to improve care and outcomes in patients with advanced and serious illness was undertaken with the objective of evaluating their effectiveness on patient-centered outcomes relative to palliative care.

 

Measuring What Matters In Hospice And Palliative Care

This effort aimed to recommend a concise portfolio of valid, clinically relevant, cross-cutting indicators for internal measurement of hospice and palliative care.

 

Achieving Excellence In Biopsychosocial Cancer Pain Management Through A Comprehensive Quality Education Program

This project will implement and evaluate a national curriculum for training cancer health care professionals in biopsychosocial pain management and quality improvement.

 

Assessment Tools For Palliative Care

The aim of this project is to evaluate the evidence base for palliative care assessment tools. Based on a conceptual framework and evidence map, this project will summarize the evidence for use of palliative care assessment tools.

 

Evaluation Of Implementation Of Point-of-care Palliative Care Quality Measures

The goal of this project is to evaluate the implementation of quality measure initiatives into quality improvement projects from the provider, administrator and patient/family perspectives.
