Dr. Joseph Finkelstein, Welch Center Core Faculty member, has been awarded a competitive contract by the Veterans Health Administration (VHA) Innovation Program entitled “Evidence-Based Informatics Framework for Complex Chronic Care.” His project will provide an Evidence-Based Informatics Framework for Complex Chronic Care in support of the VHA Innovation program.

Dr. Finkelstein will develop an innovative platform of the Chronic Disease Home Automated Telemanagement (CD HAT) system composed of 1) a web-based guide to self care, 2) a Decision Support Server, and 3) computerized disease management and care coordination tools for an eCare team. The project will bring new electronic tools to the management of chronic diseases, and its products will be compatible with the VHA’s existing electronic health information systems and patient records.
