The Center for Community HEALTH

The Center for Community HEALTH is a Community Health Worker (CHW) program created as a project of the Urban Health Institute. The HEALTH in the name reflects our focus: Health, Education, Advocacy, Leadership and Training at the Hopkins Urban Health Institute.

CHWs are used in Baltimore, across the country and around the globe in a variety of settings. Our program draws on the experience and wisdom of several East Baltimore community-academic research partnerships. We hope to add to that knowledge as we serve the people and families of East Baltimore.

CHWs in our program serve as a bridge between the individual and the health care system. The list of services we provide is only part of our story. Our goal is to form relationships with the people we serve in order to help our neighbors improve the quality of their health, while at the same time starting local residents on a career path in the health professions.

The CHW program began in January of 2005 with seven AmeriCorps members. As an AmeriCorps Maryland service site, our program works in partnership with the Governor’s Office on Service and Volunteerism. AmeriCorps members are neither volunteers nor University employees: They are neighbors committed to making a difference by giving a year of their lives to community service. An AmeriCorps grant provides the bulk of the funding for a modest living allowance.

During our first two years of operation, CHWs have served more than 1,700 residents and become an integral part of the health care team in East Baltimore. CHWs meet community members through both group and individual outreach and in the clinical setting, by working with the East Baltimore Medical Center to improve maternal and child health.

Our services Include:

  • Health risk assessments
  • Blood pressure screenings
  • HIV rapid testing and counseling
  • Health education (individual and group)
  • Health coaching
