Student Dental Insurance

Optional - Read Carefully!

Open enrollment for our two dental plans is July.  Continuing students must enroll July 1 to June 30. Your yearly premium will be posted to your account and will be due on July 17, 2009.

New students have the option of enrolling during in-person registration.  Summer deadline - July 10, 2009 with coverage effective August 1, 2009.  Fall deadline - September 11, 2009 with coverage effective October 1, 2009.  Yearly premiums will be posted to their account at that time.

Students cannot terminate their dental coverage once enrolled for the year.

Plan I:  DHMO

Monthly Insurance Rates

  • Individual                       $16
  • Husband & Wife               $32
  • Individual & child(ren)       $32
  • Family (includes spouse)   $46

Plan Summary

  • pick a primary dentist (http://www.ucci.com/was/ucciweb/home.jsp)
  • co-payment for each service stated in brochure - no questions on price (see co-pay list)
  • all class I services have $0 co-pay
  • $5 co-pay for office visit
  • all class II and III have higher co-pays
  • no claims – pay at visit
  • no deductible or maximum       

Plan 2:  Access Plan

Monthly Insurance Rates

  • Individual                       $12
  • Husband & Wife               $21
  • Individual & child(ren)       $27
  • Family (includes spouse)   $40

Plan Summary

  • no primary dentist - larger network (http://www.ucci.com/was/ucciweb/home.jsp )
  • all class I services are covered 100% up to $500 maximum annually
  • all class II and III are discounted 20-30% off usual cost varying by dentist
  • no brochure with pricing – you must contact individual dentist for their rates
  • no claims – pay at visit
  • $25 annual deductible
