All Full-time On-Campus Students Only  - $450.00 annual fee (July 1, 2011 - June 30, 2012)

JHSPH Student Health Plan Monthly Insurance Rates - Academic Year 2011-2012 (effective July 1, 2011)   

Individual:  $256.00
2-Party:     $632.50  
Family:      $845.00 

Requirements
All full-time and international students are required to purchase health insurance.  To opt out of purchasing the JHSPH Student Health, students need to provide proof of comparable coverage.  Comparable coverage would need to provide a minimum of $250,000 US in coverage for all medical costs and allow the student to use the insurance in the State of Maryland.  Contact the JHSPH Student Accounts Office to request a waiver form to opt out of purchasing the Student Health Plan.  Students will need to show proof that they have purchased a comparable insurance policy (i.e. ID card with effective date or other documentation with their name it).  Student Accounts can be contacted at 410-955-5725 or bursar@jhsph.edu and they are located in W1101 Wolfe St. Bldg.

Please note:  All Full-time JHSPH students are billed the $450.00 Health Clinic Fee regardless of whether they have purchased the JHSPH Student Health Plan.
