Access Plan Summary

• No primary dentist—larger network (view list under Concordia Access on United Concordia website) 
• All class I services are covered 100% up to $500 maximum annually
• All class II and III are discounted 20–30% off usual cost varying by dentist
• No brochure with pricing—you must contact individual dentist for their rates
• No claims—pay at visit
• $25 annual deductible
