SUBJECT: Procedures
for Affirmative Action Report Form
POLICY
It is the policy of the Bloomberg School
of Public Health to openly advertise the credentials for available full-time
professorial faculty appointments in a special effort to attract, among other
qualified candidates, qualified candidates who are members of minority groups
and women.
To implement this policy, the Dean will
submit for the approval of the Advisory Board only those new full-time
professorial faculty appointments for which an Affirmative Action Report Form
has been completed by a department head or a head of a search committee.
PROCEDURE
The Affirmative Action Report is to be
completed by department heads and search committees. The Dean is responsible for ensuring that no professorial faculty
appointments are approved by the Advisory Board without an appropriately
completed Affirmative Action Report Form.
The principle of open recruitment
requires that efforts be made to make the availability of the position and required
credentials widely known to prospective candidates, especially those who are
members of minority groups and women.
Advertisement is one means of open recruiting and such advertisements
should at a minimum indicate that the University is "An Equal Opportunity/Affirmative
Action Employer." A
description of the position should be available to all candidates. All reports are subject to review by the
Dean and the Affirmative Action Committee.
Each section of the Affirmative Action
Report must be completed for all full-time professorial faculty
appointments including professors, research professors, public health
professors, associate professors, associate research professors, associate
public health professors, assistant professors, assistant research professors,
and assistant public health professors.
This form must be completed for instructors who are recruited with the
intention of filling a tenure-track position.
Any additional information which pertains
to the candidate selected should be submitted in Section 9 on "Comments
or Special Justification."
A
description of the position should be made equally available to all candidates.
All completed reports are subject to
review by the Dean, appropriate staff, and the Committee on Affirmative
Action. Completed reports are
maintained in the Dean's Office.
A
copy of the current Affirmative Action Report form dated December 2000, is
attached.
