Enhancing the Quality of "Medical Home" Services

The Lipitz Center and its partner organizations are providing technical assistance to medical practices that wish to transform into medical homes or accountable care organizations. Several forms of technical assistance are available, including practice self-assessment; "Practice Leaders in Medical Homes" online course; "Guided Care Nursing" online course; Guided Care implementation manual; and information about selecting health information technology.

Guided Care: Integrating High Tech and High Touch

The Johns Hopkins Bloomberg School of Public Health is conducting a rigorous test of Guided Care at eight sites in the Baltimore-Washington DC area. This randomized study will measure the effect of Guided Care on patients’ and families’ quality of life, as well as on overall health care costs and primary care practices. Consultants and national leaders in health policy, consumer advocacy, health insurance, and health care delivery are helping shape the study and, if the results are favorable, will speed the adoption of Guided Care throughout American health care.

The Effectiveness of Interdisciplinary Primary Care for the Elderly: an Evaluation of Cost and Utilization Outcomes

The purpose of this study is to evaluate the care provided by the Senior Health Network (SHN) clinics of Baylor Health Care System to Medicare beneficiaries.  The SHN clinics have an interdisciplinary staff and a model of care that incorporates a social work visit for every new patient, chronic care clinics, and health education.  Using Medicare claims data, cost and utilization outcomes are being compared for a cohort of SHN patients and a similar cohort in a network of traditional primary care clinics.

A Randomized Trial of Training and Support for Voluntary Caregivers

The goals of the project are to conduct and evaluate a multi-component, community-based intervention for caregivers who provide assistance to older adults with complex health-related needs. The intervention combines training and support for caregivers within the context of an innovative model of health care (“Guided Care”) designed to facilitate coordinated, evidence-based health care for frail older persons.

National Demonstration and Evaluation of the Johns Hopkins Home Hospital Model: Training, Coordination, and Technical Assistance

Coordination of Primary Care Referrals to Gastroenterologists in Northern Israel

Principal Investigator: Efrat Shadmi, RN, PHD
Supported By: The University of Haifa Research Foundation

This study examines how referrals of patients from primary care physicians to gastroenterologists are coordinated in terms of inter-provider communication and alignment of expectations and objectives between the three stakeholders (primary care physicians, gastroenterologists, and patients). The effects of coordination on referral results are being explored.
