SECTION CC: LIFETIME MENTAL HEALTH AND HEALTH BEHAVIORS

This part of the interview asks about your physical and emotional well-being over your whole life, including now. It is important for us to get accurate information. In order to do this, you will need to think carefully before answering the following questions.

 

CC2. Have you ever had a period of one month or more when most of the time you felt worried, tense or anxious about everyday problems such as work or family?

CC2a. People differ a lot in how much they worry about things. Did you ever have a time in your life when you worried a lot more than most people would in your situation?

CC3. Now please think about period of feeling sad, empty or depressed. In your lifetime, have you ever had two weeks or longer when nearly every day you felt sad, empty, or depressed for most of the day?

CC4. In your lifetime, have you ever had two weeks or longer when you lost interest in most things like work, hobbies, or other things you usually enjoy?
