Obtaining a license to Use the PraPlus™

Most organizations that request licenses to use the PraPlus™ (rather than the Pra™) intend to administer it directly to members of older populations for which they provide health care.

The license fee is based on the number of licenses requested.  One license is required for each legal entity, e.g. state - licensed insurance company.

The duration of the agreement is 3 years.

License Agreement

If your organization decides to obtain a license to use the PraPlus™, you should complete the following steps:

  1. Print the License Agreement (pdf file).
  2. An authorized representative of your organization should complete and sign the Licensing Agreement.
  3. Write and sign a check (made out to Johns Hopkins University) corresponding to the correct amount needed to cover the three-year licensing royalty fee.
  4. Send the Licensing Agreement and the check to:

Upon receipt of full payment and your properly executed license agreement, we will send you the PraPlus™ instrument and a copy of the signed license agreement. Then you may begin using the PraPlus™.  If you have not received notification from us within 5-7 business days, please contact Adriane King at (410)955-6546, or by email at aking@jhsph.edu.

The Pra™ and the PraPlus™ are protected by copyright. Any use of these instruments must be licensed.
