To fill in the form, open the file by double-clicking on the jhupr.dot file. A new document is created and the cursor is located on the first field of the form. After completing an entry, tab to the next field (shift-tab to return to previous field). Each field has a character limit but you can include further information by tabbing to the end of the document and filling out the Additional Comments section on page 2. Since this is now a document file, you can save it using any name that is meaningful to you. E-mail or otherwise send to your department administrator. |
Run WordPerfect and open the file. Select Tools, Merge, Merge and select Current Document as Form file. Select Merge to begin filling in the fields. A prompt will display for each field. Click on the Continue button or press Alt-Enter to move to the next field. Click on Quit to merge the form. Since this is now a document file, you can edit it, print it, and save it using any name that is meaningful to you. Important:Be sure that the form itself prints one page. Put overflow information on page 2 under Additional Comments if necessary. E-mail or otherwise send to your department administrator. |
Click on the above Acrobat link. When the Save As ... dialog box appears, save the file as "jhupr.pdf" to your hard drive. To complete the form, double-click on the file after downloading. Netscape/Communicator 4.x Users Click on the above Acrobat link to display the form inside your browser. All Users The hand tool appears and as you move your mouse over a fillable area, the hand becomes a text cursor. Begin entering text in that field and tab to move to the next field. Press shift-tab to move to previous fields. There is a blank page where you can enter additional information. Press PageDown to access page 2 of the form. Currently, you cannot save the form along with its data in Acrobat Reader. You need to print the form and send to your department administrator. |
