Online Personnel Requisition

The Johns Hopkins University School of Public Health Divisional Human Resources Office is pleased to announce that the Personnel Requisition form is now accessible for use via our Web site (for internal use only). Click on http://www.jhsph.edu/hr/instructions.htm to go to the form.

You have the option of completing the form in one of three ways:

  • Microsoft Word
  • WordPerfect
  • Adobe Acrobat. (If you choose this option, you will need to download the software. Questions regarding this process should be directed to the SPH Information Services Help Desk at 410-955-3781.)

Please note:  For some positions the Compensation Unit in Human Resources has established online generic job descriptions. If applicable, these generic job descriptions can be attached to a requisition to document and explain the duties and responsibilities of the job. The generic job descriptions can be accessed through the University's Web site at http://www.jhu.edu/~hr1/compensation/gjd.html.

Questions regarding the content of the Personnel Requisition form should be discussed with the Compensation Unit at 410-955-0775. Completed online Requisition Forms including any additional documentation should be submitted to Compensation by one of four methods:

  1. Electronically, as an e-mail attachment through your department's Administrative office to comp@jhsph.edu
  2. Manually, by printing the completed copy, obtaining required signatures and faxing it to the Compensation Unit at 410-614-3805,
  3. Manually, by printing the completed copy and mailing it by interoffice or US mail, to Compensation at 2021 E. Monument St., Baltimore, MD 21205,
  4. Manually, by printing the completed copy, obtaining required signatures and hand delivering to Compensation at the same address.

Please retain a copy of the requisition for your records and to refer to, when working with Compensation or Employment staff.

We hope the move of the Personnel Requisition Form to the JHSPH Web site will make it more convenient and give you greater flexibility when submitting this document to Human Resources.
