Course Registration

Course registration is limited by course.  A completed registration form, along with a full fee payment is required to secure your space; registration forms will not be processed without payment.  Preference will be given to Certificate Program participants.  You will receive an e-mail confirmation regarding registration. 

1. Certificate Program candidates (non-credit) - Please register on-line.  
2. Johns Hopkins Degree-Seeking Students - All JHU degree candidates registering for academic credit must register via the Office of Records and Registration web registration services at https://isis.jhu.edu/sswf/.
3. Non-Johns Hopkins Degree-Seeking Students - If you wish to take a course as an elective for academic credit, you should register as a special student limited, following the directions that appear on this registration form. Payment and instructor’s signature must be submitted with the form.  

***Registration for the Database Design and Implementation in Clinical Research Course – enrollment is limited to 20 computer lab positions; therefore all registrations must be completed using this registration form.
