Student Assembly Awards Official Nomination Form

Nominations are now being accepted.
Deadline:  March 30, 2011

Please fill out a different on-line form for each person you are nominating. You may nominate as many people as you like, even for the same award. Please click here to see a list of people not eligible for this year’s awards. ONLY STUDENTS can nominate individuals for any of the awards. Professors are not eligible to nominate anyone for any of the awards.

All nominated persons are notified and congratulated for the nomination, even if they do not win the award. Nominees are not informed of the names of students who nominated them unless the Student Assembly receives direct permission from the students to have their names disclosed.

Note that award winners are selected based on the content of your statement. While conciseness is appreciated, the more you can say about the person and the various reasons she or he deserves to win, the better chances your nominee will have. Please use the following criteria for the awards to determine what information to include in your nominations:

1:  Teaching/Mentoring ability

2:  Availability to students (easily reachable and approachable, available for advice or help, etc)

3:  Improving education or quality of life at the School (going above and beyond job description for students, etc)

4:  Professionalism / friendliness (overall character of the nominee as described by the nomination statement)

1:  “Above and beyond” factor (extra effort and work extended to be of help to students, etc)

2:  Knowledge (ability to give appropriate help, information, or direction to students, etc)

3:  Availability / “student connection” (person is accessible, responsive, always thinking of students, etc)

4:  Professionalism / friendliness (overall character of the nominee as described by the nomination statement)

TEACHING ASSISTANT Awards

STUDENT RECOGNITION Awards

1:  Knowledge of subject material

2:  Teaching ability (ability to explain material clearly, patience & demeanor in teaching, etc)

3:  Availability to students (going above and beyond job description, help outside of class, etc)

4:  Professionalism / friendliness (overall character of the nominee as described by the nomination statement)

1:  “Above and beyond” factor (contributions were original, greatly needed, beyond what most students would or could do, etc)

2:  Time and effort (amount of time and dedication contributed to improving life at the school or community)

3:  Availability / assistance to other students (person is accessible, responsive, helpful to other students, etc)

4:  Professionalism / friendliness (overall character of the nominee as described by the nomination statement)
