- Summary of Health Benefits for Postdoctoral Fellows and Students -- This document works in conjuction with the Summary Plan Description (above) to further explain your benefits. It also contains a list of handy telephone numbers and a map of campus showing all the locations relevant to our health care. Please note that the UHS clinic has moved since this book was developed... it is now located at 401 N. Caroline St. in the building labeled on the map as "Bond Street Plaza" (at the corner of Caroline and Orleans).
Update -- This document gives updated information on services that have changed since the printing of the 2006-07 version of the Summary of Health Benefits document above. These changes will be incorporated into the 2007-08 version of that booklet.
Mental Health fact sheet -- This document gives additional information regarding available Student Mental Health services.
Medical Records Release Form -- This form will allow another health care provider to release your medical records to you or to your new doctors at UHS. At the top of the form, you determine which records are released (e.g. "blood tests," "X-rays," or "all"). You also must state which doctor is releasing the records, and to whom you are sending them. Many people choose to have their records sent to themselves, and then take them to UHS personally. If you choose to have the records sent to UHS directly, fill in the details as follows:

Medical Claim Form -- Use this form to request reimbursement for medical expenses that are covered by SHP, but for which you paid out-of-pocket. Additional details about filing claims can be found in the Summary Plan Description on pages 38, and 48-57.
